Board staff cannot provideВ an applicant with a theory testing center'sВ schedule. The testing centers determine the schedule for their facility. Theory Examinations are held in multiple locations with dates and times that are specificВ to each location.

Theory Candidate Information Bulletins (CIB) list sample questions for each license discipline. The applicant will be provided a link via email following submission of their application to schedule the specific location, date, and time that is convenient for the applicant.ДЄpplicants must pass the theory examination before application and fee for the practical examination may be submitted for scheduling. TheoryВ scheduling will be completed by the applicant after applying with the board office. Applicants MUST provide a valid email address in order to schedule the theory examination location, date, and time.

Beauty Culture Provisional Verification:Section A - Experience in Your Jurisdiction and the Indiana Supervising Licensing and Beauty Culture Salon Sections are required for Provisional License Applicants.Provisional License Applicants and Upgrade Applicants:.Verification of Licensure: We must receive verification from every state, territory, and country that you hold or have held a license.Examination Pass Notice: You must upload your examination pass notice or submit to our office as part of your application.Beauty Culture Education Certification: You must have your Indiana approved beauty culture school complete the Beauty Culture Education Certification form and upload the document as part of the application process.Submit copies of court documents for each instance to support the statement. Include the violation, location, date and disposition. Positive Response Documentation: If you answer 'Yes' to any disciplinary/background questions on the application, explain fully in a statement that includes all details.

Documentation may include a copy of your marriage certificate or divorce decree.

Name Change Documentation: Documentation of any legal name change if your name differs from that on any of your documents.

Additional Documentation: Documentation may be uploaded as part of the online application process, or you may log back into your account at MyLicense.IN.gov and use the License Update feature to submit documentation after your initial application has been submitted.

Application Fee of $40.00: Pay by credit or debit card for applications submitted online at MyLicense.IN.gov.
